Healthcare is one of the most important practical considerations for families moving to Spain — and one of the most misunderstood. This guide covers public vs private healthcare, costs, English-speaking doctors, paediatric care, and what to arrange before you move.
Public healthcare (SAS)
Spain's public healthcare system is well-funded and generally high-quality. Access for residents is through social security contributions (your employer pays, or you pay as autónomo). EU citizens can use the S1 form for temporary cover. Waiting times for specialists can be long.
Private healthcare
Most expat families carry private insurance (€100–€200/person/month) for faster access, English-speaking doctors, and more convenient appointment times. Major providers include Sanitas, Adeslas, and Cigna. Private health insurance is also required for most visa applications.
Paediatric care and emergencies
Paediatric care is good on the Costa del Sol. Hospital Costa del Sol (Marbella) and Hospital de Fuengirola both have paediatric departments. English-speaking GPs are available in all major towns.
